Job Responsibilities Assist the participant to develop a Person-Centered Plan to help them achieve their life goals.
Conduct home/community visits with the participant, their family, their team.
Coordinate and facilitate team meetings with the participant, their family and community providers.
Accurately document all case management activities daily in LTSS.
Be knowledgeable of and follow all State, Federal and DDA guidelines.
